How to make a referral

Referral criteria
To be accepted by the service, people must be:
- Individuals aged 18 and over
- Individuals who are registered to a Norfolk and Waveney GP and could be reasonably considered to be ordinary resident within the Norfolk and Waveney ICB geographical footprint
- Individuals recently admitted to hospital, or A&E or an Intermediate care bed in the community or
- Individuals who have recently experienced a health crisis and required support from admission prevention services
- Individuals who do not ordinarily or currently have friends/family that are able to provide practical support needed to prevent an admission or when a person is discharged from hospital
- Able to self-mobilise (where the support provided would require this).
We cannot accept:
- Interventions that are outside of low level urgent need
- Referrals where needs should be met by CQC regulated activities or there is an anticipated long term need for the support.
- Those who have complex needs, e.g.
- Dependency issues (drugs, alcohol)
- Hoarding (above level 3 on the Clutter scale)
- Mental health conditions that are more than low-level
- Dementia beyond mild cognitive decline
- People discharged from hospital with an active package of care in place.
- Respite support
Referrals can be refused following a risk assessment.
